Immunization Specialists from the Louisiana State Department of Health and Hospitals review the immunization records of all incoming students. We are requested to advise all parents of Louisiana's immunization policy. Students who do not meet the requirements will not be permitted to enroll at any Louisiana school and cannot attend until proper documentation is furnished.
Recommended Schedules for Immunization of Infants and Children
- Birth - HBV
- 2 Months - HBV, DTaP, IPV, Hib
- 4 Months - DTap, IPV, Hib
- 6 Months - HBV, DTaP, Hib
- 12-15 Months - DTaP, IPV, Hib, MMR, Var
- 4 Years of age - DTaP, IPV, MMR
- 11-12 Years of age - Td (MMR, Var, HBV if needed)
Please note that the Hepatitis B Vaccine (HBV) is now mandatory for all students entering a Louisiana school. Also, the Varicella (Chicken Pox) vaccination, or documented proof of a diagnosed history of the disease from a doctor, is now required for entry for all pre-K, kindergarten, and other students attending any state approved facility for the first time.
If you plan to enroll your child at Parkview Baptist School, please attach a copy of your child's updated immunization record to your registration application. Each child will need a minimum of 4-DTP, 3 Polio (last DTP and Polio after 4th birthday), 2 doses of MMR, 1 Varicella (Chicken Pox), and 3-HBV. Please make sure your child is up-to-date so he/she can start school on time.
